Medical Records Release Form

When your children have had medical services prior to coming to our office, we ask that you complete the Medical Records Release Form. Vaccine information, hospitalizations, lab-work, and other pertinent information will be requested. Please note -parents of newborns do not need to fill out a record release.

Registration Form

Parents are requested to complete our registration form. This form allows us to obtain all pertinent demographic information.

Pediatric History Questionnaire

This form is two pages, but shouldn't take too long to complete -- especially for infants. It provides essential information on past medical history, development, active symptoms, and family history.

HIPPA Consent

The federal government recently enacted legislation designed to protect your privacy. The rules are generally known as HIPPA regulations. They require us to get your consent to release information and be quite specific about the circumstances. It's a little wordy -- but it's good to have.

Have you downloaded and completed these 3 or 4 forms? You get a gold star! Please either bring them with you to the first visit, or if you wish, fax them to us before your appointment (numbers are to the right). We will get you out of the waiting room and in to see the doctor a lot quicker!
