PE Forms

Our office visits for school-aged children automatically include a complementary, standardized PE form and a copy of your child’s immunizations.  Our experience is that most schools in the area now accept this form.  If your child’s school, camp, athletic coach, etc. will not accept our standardized report, we will complete the form that they have sent to you for a $5 fee.  Your child must have been seen within the preceding 12 months for us to be able to do this.
